To book places at the BCSC Purple Apple Marketing Awards Dinner, please fill out the form below. Please note, upon receiving your booking form, you become liable for payment of the fees (please see notes).
 
  Important Information  
 
bullet Please complete the form for the required number of places. The order in which the names are listed will form your table plan which will enable BCSC to compile the guest list and name the seats at the table
bullet Purple Apple Award entrants qualify for one discounted dinner ticket per entry only - therefore please ensure that your entry confirmation code is used against one attendee booking only on the form below.
A maximum of three entries are permitted from a single centre - hence a maximum of three discounted tickets.
bullet All names must be submitted by Friday 20 April 2012.
bullet Seating will be on round tables of 10 places.
bullet The dress code is black tie.
bullet Reception drinks and half bottle of house wine per person at the dinner is included in the price of the ticket. Additional wine can be ordered from the venue on the evening, and a full cash bar will be available.
bullet Full details and dinner tickets for all guests will be sent to the person who made the booking.

Notes:
1. Confirmation and a VAT invoice/receipt will be issued in respect of all bookings. VAT is charged at the current rate of 20%.
2. All cancellations must be notified in writing to BCSC at events@bcsc.org.uk. Cancellation charges will apply as follows:
- for cancellations advised between 3 February to 30 March 2012 - 50% of the full fee will be payable
- for cancellation advised after 30 March 2012, no fees will be reimbursed and the invoice will remain payable in full.
3. Cheques and bank drafts drawn in pounds sterling on a UK bank, must be made to British Council of Shopping Centres.
4. Bank transfers should be made as follows: BCSC; NatWest; Sort Code: 60-17-21; Account no: 67410006. Please quote your organisation name and forward remittance advice to accounts@bcsc.org.uk.
5. Admission to the event will not be permitted if payment or BACS advice has not been received.
